Alloplastic Facial Implants in Aesthetic and Reconstructive Surgery | Dr. Daniel Gould

Dr. Daniel Gould co-authored this systematic review and meta-analysis published in Aesthetic Plastic Surgery, examining the outcomes, applications, and potential complications associated with alloplastic facial implants. 

Alloplastic implants are synthetic materials used to augment or reconstruct the facial skeleton. They may be used to restore facial symmetry following trauma or congenital differences, as well as for aesthetic augmentation of areas such as the cheeks, chin, forehead, and regions around the eyes.

The researchers reviewed 17 studies involving 2,100 patients, with follow-up periods ranging from one month to 27 years. They evaluated outcomes according to both the anatomical location of the implant and the specific material used. 

The analysis identified differences in complication profiles depending on implant material and placement. Mersilene mesh demonstrated the highest infection rate among the materials evaluated, while methyl methacrylate was associated with the highest rate of hematoma. Implants placed around the eyes demonstrated higher rates of inflammation, removal, and unfavorable cosmetic outcomes compared with other facial locations. 

The researchers also found that porous implants were more likely to be associated with infection than nonporous materials. Despite these differences, the overall findings supported alloplastic facial implants as a reliable option for restoring facial symmetry and augmenting the facial skeleton, with relatively low complication rates. 

The research highlights the importance of understanding how implant material, anatomical location, and individual surgical goals influence outcomes, allowing surgeons to make more informed decisions when planning aesthetic or reconstructive facial procedures.
