The greater omentum, supplied by the gastroepiploic vessels, has become an important potential donor site for vascularized lymph node transfer. The technique involves transferring healthy lymphatic tissue with its blood supply to help restore lymphatic drainage in areas affected by lymphedema.
Dr. Gould and his co-authors used computed tomographic angiography (CTA) from 34 patients to study the right gastroepiploic artery and surrounding lymph node anatomy. The goal was to establish more predictable anatomical measurements that could help surgeons plan lymph node transfer procedures.
The researchers identified the right gastroepiploic artery in 33 of the 34 patients studied. Their analysis also mapped the location and number of lymph nodes surrounding the vessel, providing practical guidance on the dimensions of tissue required to obtain multiple lymph nodes for transfer.
The findings support CTA as a valuable tool for patient-specific surgical planning before gastroepiploic vascularized lymph node transfer. By better understanding the vascular and lymphatic anatomy before surgery, surgeons can approach these complex reconstructive procedures with greater anatomical precision.
