Anatomic Basis of Gastroepiploic Vascularized Lymph Node Transfer | Dr. Daniel Gould

Dr. Daniel Gould co-authored this research published in Plastic and Reconstructive Surgery Global Open, examining the anatomy of the gastroepiploic vascular system and its potential use as a donor site for vascularized lymph node transfer in the surgical treatment of lymphedema.

Vascularized lymph node transfer is a microsurgical technique that involves transferring healthy lymph nodes together with their blood supply to an area where lymphatic drainage has been compromised. The omentum, supplied by the gastroepiploic vessels, represents one potential source of lymphatic tissue for this procedure.

Dr. Gould and his co-authors analyzed computed tomography angiography scans from ten patients to better define the anatomy of the right gastroepiploic artery and the location of lymph nodes along its course. The right gastroepiploic artery was identified in nine of the ten patients, with an average of approximately three identifiable lymph nodes within the relevant lymphatic region. 

The researchers also measured the location of these lymph nodes relative to the vascular anatomy, helping establish practical dimensions for harvesting a flap containing multiple lymph nodes. This anatomical information can assist surgeons in planning the amount of tissue and vascular pedicle required for transfer.

The findings support the gastroepiploic region as a potentially valuable donor site for vascularized lymph node transfer. The authors also identified CTA as an effective preoperative imaging tool for mapping this anatomy and supporting more precise, patient-specific surgical planning. 

The research contributes to the continued refinement of lymphatic microsurgery by providing surgeons with anatomical data that can help guide donor-site selection and operative planning for patients with lymphedema.
