Outcomes of Soft Tissue Reconstruction for Lower Extremity Fractures with Compromised Vascularity | Dr. Daniel Gould

Dr. Daniel Gould co-authored this research published in Plastic and Reconstructive Surgery Global Open, examining how arterial injuries affect reconstructive outcomes in patients with severe lower extremity fractures. 

Lower extremity fractures accompanied by vascular damage represent particularly complex injuries. In addition to repairing bone and covering exposed structures with healthy tissue, surgeons must account for compromised blood flow, which can affect wound healing, tissue survival, and the ability to preserve the injured limb.

The researchers retrospectively reviewed 158 injured extremities requiring soft tissue reconstruction, including 26 with documented arterial injuries. They evaluated factors such as the location of vascular damage, reconstructive flap requirements, number of operations, flap failure, and eventual limb salvage.

Patients with arterial injuries required larger reconstructive flaps and underwent more operations than those without vascular compromise. They were also more likely to experience associated nerve injuries, require fasciotomy, and present with fractures involving the distal portion of the leg.

Following reconstruction, arterial injury was associated with increased rates of both flap failure requiring additional surgery and amputation. Despite these challenges, limb salvage was still achieved in the majority of patients with vascular injuries. 

The findings highlight the importance of vascular status when planning reconstruction after severe lower extremity trauma. Understanding the effect of arterial injury can help surgeons anticipate complications and develop reconstructive strategies focused on preserving both tissue viability and the injured limb.
