Functional Outcomes After Lower Extremity Trauma and Limb Salvage | Dr. Daniel Gould

Dr. Daniel Gould co-authored this research published in Plastic and Reconstructive Surgery Global Open, examining long-term functional recovery in patients who required soft tissue reconstruction following traumatic injuries to the lower extremities. 

Severe lower extremity trauma can result in extensive damage to skin, muscle, bone, and other tissues. Reconstructive surgery may allow surgeons to preserve the injured limb, but successful limb salvage does not necessarily mean that patients will regain their previous level of physical function.

The researchers evaluated patients treated with soft tissue reconstruction at a Level I trauma center. Functional recovery was assessed using validated measures, including the Short Form-36 Health Survey and Lower Extremity Functional Scale, allowing the team to examine mobility, physical function, pain, and overall quality of life. 

At follow-up, the majority of patients had achieved successful limb salvage, but many continued to experience substantial functional limitations. Half of the surveyed patients were fully ambulatory, while others required assistance or remained unable to walk independently. 

The study also compared reconstructive approaches, including local and free tissue transfer. Differences in functional outcomes were observed between these groups, although the authors emphasized that further research is needed before broader conclusions can be drawn. 

The findings highlight an important distinction in reconstructive surgery: preserving a limb is only one measure of success. Understanding long-term mobility and quality of life is also essential when evaluating outcomes and guiding treatment decisions after severe traumatic injury.
