Thyroid-associated ophthalmopathy can affect the tissues surrounding the eyes, producing changes such as eyelid retraction, eye prominence, swelling, double vision, and alterations in the appearance of the eye area. Importantly, the condition can occur independently of hyperthyroidism, making accurate diagnosis particularly important.

The review highlights that thyroid-associated ophthalmopathy is frequently underdiagnosed or misdiagnosed. This is especially relevant in aesthetic practice because some patients may initially seek blepharoplasty or other periocular procedures for concerns they perceive as signs of aging, when an underlying thyroid-related condition may be contributing to those changes.

Dr. Gould and his co-authors reviewed the available literature to provide surgeons with a clearer framework for understanding the prevalence, diagnosis, pathophysiology, and management of the condition. The research noted that approximately 3% of patients who had undergone blepharoplasty may have thyroid-associated ophthalmopathy. 

The authors emphasize that blepharoplasty and other procedures around the eyes must be approached carefully in patients with thyroid-associated ophthalmopathy. Recognizing the condition before surgery can help reduce the risk of both aesthetic and functional complications.

The review reinforces the importance of looking beyond surface-level aesthetic concerns when evaluating the eye area, combining careful diagnosis with an understanding of the underlying anatomy and medical factors before determining an appropriate treatment plan.
